Chronic otitis media is a long-lasting middle ear infection that can damage the eardrum and lead to hearing loss if left untreated.

Chronic otitis media is a persistent infection of the middle ear, typically lasting longer than 3 months or recurring frequently over time. It affects not only the middle ear cavity behind the eardrum but also the mastoid air cells, which are connected to the middle ear.

This condition usually develops when an acute middle ear infection is not properly treated or fails to heal completely. Over time, the infection can become progressive and destructive, leading to complications such as eardrum perforation and structural damage within the ear.

What Is Chronic Otitis Media?

Chronic otitis media is characterized by long-term inflammation of the middle ear lining. As the infection progresses, it may cause:

  1. Perforation (hole) in the eardrum
  2. Damage to the middle ear bones (ossicles)
  3. Infection spreading to surrounding tissues
  4. In some cases, involvement of the outer and inner ear

There are two main types:

  1. Simple Chronic Otitis Media:
  2. Affects primarily the eardrum and ossicles
  3. Cholesteatomatous Otitis Media:
  4. A more serious condition where abnormal tissue growth (cholesteatoma) damages surrounding structures and may cause bone erosion

Causes of Chronic Ear Infection

The most common cause is inadequate or incomplete treatment of acute otitis media. Other contributing factors include:

  1. Recurrent ear infections
  2. Eustachian tube dysfunction
  3. Anatomical variations
  4. Weak immune system
  5. Chronic sinusitis
  6. Frequent upper respiratory infections
  7. Poor environmental conditions

Effects on the Eardrum

In chronic otitis media, the eardrum is often not intact and may have a perforation. The size and location of this perforation are important for treatment:

  1. Central perforation:
  2. Located in the middle of the eardrum, usually less dangerous
  3. Marginal perforation:
  4. Extends to the edge (annulus) and may involve more critical areas

Perforations in the upper part of the eardrum (pars flaccida) can have a greater impact on hearing.

Additionally, calcium deposits known as myringosclerosis or tympanosclerosis may develop in the eardrum or middle ear lining.

Symptoms

  1. Persistent or recurrent ear discharge
  2. Hearing loss
  3. Ear fullness
  4. Occasional pain
  5. Tinnitus (ringing in the ear)

Treatment Methods

Treatment depends on the severity and type of infection:

  1. Medical Treatment:Antibiotics
  2. Ear drops
  3. Regular cleaning of the ear
  4. Surgical Treatment:Tympanoplasty (repair of the eardrum)
  5. Mastoidectomy (removal of infected mastoid cells)

Early diagnosis and proper treatment are essential to prevent permanent hearing loss and serious complications.
